The product

A startup with a surgeon as its public face

Empathia was built by xmethod's partners together with a practicing bariatric surgeon. The doctor is both the idea behind the product and its face: more than 20 years of practice, over 1,000 surgeries, talks at international conferences, TV appearances and an established YouTube audience. That reach gave the product a warm, trusting audience from day one.

The challenge

A niche no app was built for

The brief was a food and health tracker focused on bariatric patients, but scalable to a much broader audience from the outset. Staying in the surgical niche alone would have been a real monetization risk.

  • Too many apps

    People stitch together separate apps for food, water, weight and activity.

  • No specialization

    No app on the market is made for life before and after bariatric surgery.

  • Hard to monetize

    Free models like FatSecret's cap revenue, and the niche alone is small.

Discovery

30 interviews and a niche-first strategy

We mapped the market and key competitors (MyFitnessPal, FatSecret, YAZIO) and ran 30 in-depth interviews with patients who had already had bariatric surgery. The findings: users combine several apps, more assertive paywall models earn more, and nobody serves bariatric patients specifically.

Product concept

One window for health

Instead of a maze of screens, we designed one hub: every core function lives in a chat with the AI assistant, with a separate thread for the period before and after surgery. One consistent interface, as few steps as possible to each action, and multimodal input by voice, text or photo.

  • A diary for food, water, weight and activity
  • Calorie and macro counting for every meal
  • Nutrition recommendations based on personal preferences
  • Booking a consultation with a doctor
  • Uploading and analyzing medical test results
  • Voice logging (hypothesis)

    Interviews showed people avoid photographing meals in public. Say what you ate, and the system classifies the meal and logs the macros. To be tested after the MVP launch.

  • Streaks that score behavior

    Daily, weekly and monthly streaks evaluate the user's overall pattern, not a single logged meal.

  • Wearables from the design stage

    The architecture was planned for wearables with input from a data engineer at an international wearable-device startup.

AI & RAG

An assistant grounded in medical expertise

The assistant logs food, gives lifestyle recommendations based on preferences and health indicators, analyzes uploaded medical data and suggests recipes. It is grounded through RAG in the surgeon's own material, peer-reviewed research from vetted sources and WHO data, not generic advice scraped from the web.
